As unique as communities of practice are, as differently they may kick off: They can start as a loose network of individuals who become inspired by a shared common concern by time, or they are initiated on purpose around a common concern.
But regardless how a local community of practice may have emerged, its initiation becomes easier when the COP members can share a common understanding and vision. And, if one or more members step up to facilitate the establishment and later an eye-level running of the COP.
